1. Account and cards
The account identifies the person signing in. Each card keeps its own subject, controller, relationships and permissions. Creating an account does not by itself grant access to any card data.
2. Health data
The user and every authorized contributor are responsible for ensuring that submitted data and documents are lawful and appropriate. The portal does not replace diagnosis, professional advice or emergency services.
3. Statistics and research
By accepting these terms you consent to the generation and use of anonymized data for statistics and health-related research. These uses must apply data minimization, security controls, ethical review where appropriate and reasonable measures against re-identification. This consent does not authorize identifiable personal or clinical data to be released to any researcher.
4. Withdrawal
The controller may withdraw future research use from each card's permission management. Withdrawal does not affect processing lawfully completed before withdrawal and cannot retrieve results that are already genuinely anonymous and cannot reasonably be linked back to the person.
5. Changes and contact
Each material change will have a new version and require renewed acceptance. For questions or to exercise your rights, contact contact@unid.foundation.
